The posting

Als SPV binnen het IHT-team bied je intensieve, kortdurende behandeling aan cliënten in een psychiatrische crisis. Je helpt hen zoveel mogelijk in hun eigen omgeving, zodat een opname voorkomen of verkort kan worden. Met jouw deskundigheid, betrokkenheid en daadkracht geef je cliënten weer perspectief en regie. Wil jij werken in een ervaren, betrokken team waar je écht van betekenis bent? Dan maken we graag kennis met je. Het Intensive Home Treatment (IHT)-team is onderdeel van de 24/7 crisisdienst van GGZ Noord-Noord-Holland, samen met de Spoedeisende Psychiatrische Hulp (SEPH) en het crisis aanmeldpunt. Ons team biedt kortdurende crisisbehandeling (maximaal zes weken) aan cliënten thuis of op de acute dagbehandeling. Daarnaast voeren we crisisbeoordelingen uit tijdens kantooruren en zetten we alles op alles om cliënten de best passende zorg te bieden. Ook leiden we met veel plezier toekomstige collega's op (AIOS, VIOS en SPV'ers) en investeren we in een veilig en stimulerend leerklimaat. Samenwerking staat bij ons centraal: binnen het team én met onze andere GGZ-teams en klinieken. Als SPV begeleid je cliënten tijdens een acute psychiatrische crisis en ondersteun je hen bij het hervinden van stabiliteit en eigen regie. Je werkt vooral ambulant en bent veel onderweg naar cliënten thuis. - voer je crisisinterventies en intensieve behandelingen uit; - begeleid je cliënten in hun eigen leefomgeving; - werk je nauw samen met psychiaters, verpleegkundigen en andere behandelaren; - zet je jouw expertise ook in binnen de klinieken wanneer dat nodig is; - stem je de zorg af met ketenpartners zoals huisartsen, wijkagenten, GGD, maatschappelijk werk, woningcorporaties, dagbesteding en andere GGZ-instellingen. Dat maakt jouw werk dynamisch, betekenisvol en uitdagend. De functie is voor 16-24 uur per week.
