Jij helpt cliënten om zo lang mogelijk regie te houden over hun eigen leven en ondersteunt mantelzorgers in een fase waarin veel verandert. Jij bent het vertrouwde gezicht dat luistert, meedenkt en mogelijkheden ziet. Samen met collega's, familie en het sociale netwerk rondom de cliënt zorg je ervoor dat mensen zich gehoord, gezien en ondersteund voelen. Beweeg mee met Mijzo Als Social Worker ben jij een belangrijke schakel in het versterken van het welzijn van cliënten. Je bent zichtbaar aanwezig op de locatie, bouwt relaties op en hebt oog voor de mens achter de zorgvraag. Je luistert, signaleert en ondersteunt cliënten en hun naasten bij vragen die spelen in het dagelijks leven. Je betrekt actief familieleden, mantelzorgers en het netwerk rondom de cliënt en werkt nauw samen met collega's en andere disciplines. Daarbij kijk je steeds naar wat iemand nodig heeft om zoveel mogelijk regie, welzijn en kwaliteit van leven te behouden. Daarnaast leg je verbindingen binnen én buiten Mijzo. Je onderhoudt contacten in de wijk, weet bestaande netwerken te benutten en bouwt nieuwe samenwerkingen op wanneer dat nodig is. Je voert gesprekken, denkt mee over passende ondersteuning en vertaalt signalen naar concrete acties. Door je gedeeltelijke inzet op de werkvloer verbind je welzijn en zorg op een natuurlijke manier.
