Je helpt als thuisondersteuner in de Ronde Venen cliënten om zo prettig en zelfstandig mogelijk thuis te wonen. Geen dag is hetzelfde. Je komt bij verschillende cliënten over de vloer en sluit steeds aan bij wat iemand op dat moment nodig heeft. Je ondersteunt onder andere bij: Persoonlijke verzorging; - Huishoudelijke taken en praktische zaken in huis; - Structuur en begeleiding gedurende de dag; - Activiteiten, zoals een spelletje, knutselen of een gezellig gesprek; - Het begeleiden van cliënten naar activiteiten of dagbesteding. Jouw persoonlijke aandacht maakt het verschil. Je kijkt goed naar wat de cliënt nodig heeft en neemt zelf initiatief. Je: - Zorgt voor een veilige, prettige en vertrouwde sfeer bij de cliënt thuis; - Hebt oog voor veranderingen in gezondheid, gedrag of leefomgeving en onderneemt actie als dat nodig is; - Denkt mee over praktische oplossingen en ziet waar het huishouden makkelijker of slimmer kan; - Houdt contact met familieleden en collega's en deelt belangrijke signalen; - Werkt nauw samen met het wijkteam; - Sluit aan bij overleggen, zodat de ondersteuning goed op elkaar is afgestemd. Kortom: je biedt praktische ondersteuning, maar vooral ook aandacht, gezelligheid en een vertrouwd gezicht over de vloer. De werkdagen zijn in principe op maandag, woensdag en vrijdag van ongeveer 7.30 tot 15.30 uur. We vragen wel flexibele inzetbaarheid.
