Transferverpleegkundige UMCG , Groningen Vakgebied Verpleegkunde Functie Overige beroepen verpleegkunde Branche Ziekenhuis Aanstelling Tijdelijk met uitzicht op vast Plaatsingsdatum 30 september 2026 Niveau HBO Ervaring Ervaren Dienstverband Parttime Wat ga je doen Als transferverpleegkundige zorg jij ervoor dat patiënten na hun verblijf in het UMCG op de juiste vervolgplek terechtkomen. Jij overziet het geheel, brengt partijen samen en organiseert passende vervolgzorg van thuiszorg tot revalidatie of langdurige zorg. Geen dag is hetzelfde. Je start met het beoordelen van nieuwe en lopende aanvragen en bepaalt waar jouw inzet die dag het meeste verschil maakt. Je schakelt continu tussen patiënten, afdelingen en externe zorgpartners en houdt daarbij altijd oog voor kwaliteit en snelheid. Het werk vindt plaats in een dynamische omgeving met veranderende wet- en regelgeving, capaciteitstekorten bij ketenpartners en steeds complexere transferaanvragen. Je houdt je onder andere bezig met: - beoordelen en coördineren van vervolgzorgtrajecten; - afstemmen met patiënten, naasten en zorgprofessionals; - samenwerken met interne en externe ketenpartners zoals thuiszorgorganisaties, ZorgnaZorg en andere instanties; - opstellen van zorgaanmeldingen en zorgvuldig vastleggen in EPIC; - bespreken van complexe casuïstiek op verpleegafdelingen; - bijdragen aan verbetering van processen en kennisdeling binnen het UMCG. Je bent niet alleen organisator, maar ook adviseur, verbinder en expert op het gebied van transferzorg. Werkomgeving Het Transferpunt van het UMCG organiseert de vervolgzorg voor klinische en poliklinische patiënten van het gehele ziekenhuis, inclusief het Centrum voor Revalidatie (Beatrixoord) en het Universitair Centrum voor Psychiatrie.
