Wijkverpleging Verpleegkundige (De HBS) Aan de rand van het oude centrum van Kampen ligt De HBS. De HBS is de ideale locatie voor senioren met een langdurige zorgvraag, om comfortabel en sfeervol te wonen. Daar kunnen mensen zich écht (samen) thuis voelen en kunnen zij het leven leiden zoals het hen past! Wil jij in De HBS bijdragen aan de invulling van de zorg van morgen? Als verpleegkundige bij De HBS ondersteun je bewoners in hun dagelijkse activiteiten en help je hen hun zelfstandigheid terug te winnen en te behouden. Je werkt nauw samen met de bewoners, hun naasten en andere zorgprofessionals om zorg en ondersteuning te bieden die aansluiten bij de individuele behoeften en voorkeuren van de bewoners. Je werkt "met je handen op de rug". Wanneer een bewoner iets echt niet zelf kan - ook niet met hulp van naasten en zorgtechnologie of hulpmiddelen - staat het HBS-team klaar. Samen met je collega's ondersteun, coach en faciliteer je de bewoner. Je begeleidt de bewoner en/of hun netwerk in het leren, het zelf weer te doen (reablement). Samen met je collega's van het HBS-team waarborg je de kwaliteit van zorg door: - zorg te verlenen aan bewoners volgens het persoonlijk plan en de visie van De HBS; - bewoners en hun netwerk te begeleiden en te coachen in zelfredzaamheid; - de kwaliteit van zorg en de visie van De HBS te bewaken en te bevorderen; - mee te denken over nieuwe ontwikkelingen en zorggerelateerde vraagstukken; - verpleegtechnische handelingen uit te voeren; - coördinerende taken uit te voeren en overstijgend mee te denken; - eindverantwoordelijke diensten te draaien en werkbegeleiding te geven.
