Ook wanneer dementie het leven ingewikkelder maakt. We zoeken daarom een verpleegkundige die verder kijkt dan de zorgvraag alleen. Iemand die bewoners ondersteunt om zoveel mogelijk zichzelf te blijven, eigen regie te houden en betekenisvolle dagen te beleven. En die samen met collega's, bewoners en naasten steeds opnieuw kijkt naar wat wél mogelijk is. Jouw nieuwe baan als Verpleegkundige Op Walraven heten we welkom en geven we thuis. Aan bewoners én aan elkaar. Walraven is een kleinschalige PG-locatie waar mensen met dementie wonen. We werken vanuit de gedachte van reablement: we ondersteunen bewoners om zo zelfstandig mogelijk te blijven en betrekken familie en naasten actief bij de zorg. Geen dag is hetzelfde en juist dat maakt het werk afwisselend en waardevol. Als verpleegkundige ben je een belangrijke schakel binnen het team. Je verleent en coördineert verpleegkundige zorg aan bewoners met een psychogeriatrische zorgvraag en bent gekoppeld aan één of meerdere woningen, waar je ook meedraait in de dagelijkse zorg. Je stelt zorgplannen op, evalueert deze en onderhoudt contact met familie, mantelzorgers en andere betrokken professionals. Daarnaast begeleid je leerlingen en stagiaires, coach je collega's in de dagelijkse praktijk en draag je actief bij aan de kwaliteit van zorg. Je voert triage uit, neemt waar nodig verpleegkundige beslissingen en draait verpleegkundige diensten op Walraven. Af en toe spring je ook bij op één van de andere locaties op het Baron van Wassenaerpark.
