Jij kan iemand z'n dag maken! In Locatie De Wieken hebben we 4 woongroepen waar clienten met een vorm van dementie kleinschalig samenwonen. In elke straat tref je 7 appartementen met aan het eind van de gang een gezamelijke woonkamer. Hier treffen de clienten en medewerkers elkaar. Een huiselijke leefomgeving waar men met elkaar een zo normaal mogelijk huishouden vormt. Jij biedt zorg indien nodig, maar voorop staat de huiselijkheid. Wanneer het leven al zo onzeker is, is het gevoel van een thuis en het behouden van geborgenheid heel belangrijk. In de Wieken werken we met 'leefcirkels'. Hiermee blijft de bewegingsvrijkheid van bewoners zo groot mogelijk. Je maakt samen met de cliënt een geschikte leefomgeving, die zoveel mogelijk aansluit bij de cliënt en bij wat hij/zij prettig vindt. Jij begeleidt hem/ haar hierin. Belangrijk hierbij zijn eigen regie en zelfredzaamheid, herstel en kwaliteit van leven van de cliënt. Je ondersteunt de eigen regie van de cliënt en familie, gericht op het behouden of verbeteren van het dagelijks functioneren. Naast jouw directe collega's, zijn er een zorgcoördinator en manager om op terug te vallen. Een fijn idee!
