Wat ga je doen? Werken in ons wijkteam geeft veel uitdaging en afwisseling. Je hebt te maken met verschillende doelgroepen, vormen van zorg en diverse ziektebeelden. De ene keer ondersteun je een cliënt met een dwarslaesie en de volgende keer help je iemand na een operatie met wondzorg. Je ziet en ondersteunt mensen met dementie en mensen met fysieke beperkingen. Geen dag is hetzelfde!

In de wijkverpleging heb je één-op-één contact met jouw cliënten. Hierdoor leer je elkaar goed kennen. Je betekent iets voor iemand en dát maakt het werk zo mooi.

Samen met de regie(wijk)verpleegkundigen en de manager wijkzorg zet jij je ervoor in dat onze cliënten op een fijne manier thuis kunnen blijven wonen. Hierbij stimuleren we de zelfredzaamheid van onze cliënten: wat kan iemand nog zelf? Hoe kunnen zijn of haar naasten helpen? Kan de inzet van hulpmiddelen of slimme zorg als beeldzorg een bijdrage leveren?

Jij werkt volgens het ZorgLeefplan en helpt onze cliënten met hun dagelijkse rituelen, zoals het opstaan, aankleden, eten en drinken. Je zorgt ervoor dat cliënten hun medicatie krijgen en je verricht verpleegtechnische handelingen, zoals het verzorgen van wonden en het geven van injecties. Je hebt altijd oog voor het welzijn van onze cliënten. Natuurlijk houd je een vinger aan de pols en ben je alert op wijzigingen op iemands welbevinden. Heeft iemand meer zorg nodig? Dan overleg je met de regieverpleegkundige; samen zorgen jullie ervoor!

Ben jij ambitieus en wil je meer uitdaging, dan heb je bij ons de mogelijkheid om naast deze functie de rol van zorgcoördinator te vervullen. Ook kan het invullen van een achterwachtdienst tot jouw taken behoren. Hiervoor is het wel noodzakelijk dat je de organisatie eerst goed leert kennen. Na een zorgvuldig inwerktraject bekijken we samen met jou of je dit wilt en of je hier klaar voor bent.
