Jouw baan Een fijne dag zit vaak in de kleine dingen: een goed gesprek, een passende activiteit of een moment van herkenning. Als Welzijnsmedewerker (MBO niveau 4) zorg jij ervoor dat de psychogeriatrische (PG) bewoners in het verpleeghuis iedere dag betekenisvolle momenten ervaren. Je kijkt verder dan een activiteit alleen en vertaalt het levensverhaal en de wensen van bewoners naar een welzijnsaanbod dat écht bij hen past. Zo lever je iedere dag een waardevolle bijdrage aan hun kwaliteit van leven. Jouw rol: In deze functie ben jij dé specialist op het gebied van welzijn binnen de locatie. Je werkt methodisch en vertaalt het levensverhaal, de wensen en behoeften van bewoners naar concrete welzijnsdoelen in het zorgleefplan. Je organiseert en stemt af met de coördinator en het welzijnsteam over een gevarieerd welzijnsprogramma voor bewoners met een complexe zorgvraag, zowel individueel als in groepsverband. Daarnaast ben je een inspirerende welzijnsmedewerker in samenwerking met zorgmedewerkers en huiskamerassistenten. Je laat zien hoe welzijn verweven kan worden in de dagelijkse zorgmomenten en stimuleert collega's om hier actief aan bij te dragen. Je beheert de beschikbare welzijnsuren, bewaakt de kwaliteit van het aanbod en zorgt voor duidelijke rapportages in het zorgleefplan.
