Deze vacature Wijkconsulent Opvoeden en Opgroeien (28-36 uur) is jouw volgende stap als je: - wilt werken bij een groeiende en bloeiende netwerk organisatie, waar de kansen voor het oprapen liggen - je ambities waar wilt maken - aan de slag wilt in een divers en professioneel team Het wijkteam Samen055 is laagdrempelig aanwezig in vijf stadsdelen in Apeldoorn. Inwoners kunnen bij het wijkteam terecht met vragen op alle levensdomeinen. Dit ga je doen Als consulent van het wijkteam luister je, vraag je door en onderzoek je hoe gezinnen en jeugdigen zelf weer verder kunnen. Soms met hulp van het netwerk, soms met hulp van lichte of specialistische ondersteuning. Je kijkt daarbij breder dan alleen opvoeden en opgroeien. Je bent in staat om vanuit meerdere perspectieven te komen tot passende hulp. Hulp en of ondersteuning die zich richt op het vergroten van de zelfredzaamheid en het versterken van het sociale netwerk van het gezin. Je bent goed op de hoogte van de sociale kaart van Apeldoorn en weet deze op de juiste wijze in te zetten. Concreet betekent dit dat jij: - intakes bij gezinnen thuis - hulpvragen breed in kaart brengt - luistert, advies geeft, het netwerk betrekt, oplossingsgericht werkt en beoordeelt of er wel of niet professionele hulp ingezet moet worden - wanneer van toepassing het gezin doorgeleidt naar voorzieningen binnen Samen055 of samenwerkende partners. - niet zelf uitvoerend hulpverlener bent, maar een regierol vervult.
