Een cliënt heeft thuis meer zorg nodig dan een paar weken geleden. De familie maakt zich zorgen en ook de huisarts wil graag met je overleggen. Je luistert, analyseert de situatie en bepaalt wat nodig is. Misschien moet de zorg worden aangepast, misschien is extra ondersteuning nodig of moet er een andere zorgprofessional worden ingeschakeld. Dat is precies wat jouw rol als wijkverpleegkundige zo waardevol maakt: jij bent de spil rondom de cliënt. Als wijkverpleegkundige werk je zelfstandig in de thuiszorg in de regio Den Helder. Je hebt een verantwoordelijke en coördinerende rol en krijgt te maken met uiteenlopende en soms complexe zorgvragen. Je kijkt daarbij verder dan alleen de medische situatie. Hoe kan iemand zo zelfstandig en prettig mogelijk thuis blijven wonen? Dat is de vraag die steeds centraal staat. Je wordt onderdeel van een professioneel thuiszorgteam in de regio Den Helder. Je werkt zelfstandig en hebt veel eigen verantwoordelijkheid, maar kunt altijd terugvallen op collega's. Samen kijk je naar complexe situaties en zoek je naar praktische oplossingen. Binnen het team is ruimte voor initiatief. Je bent bovendien niet alleen degene die zorg coördineert. Je bent ook een belangrijke vraagbaak en inhoudelijk coach voor je collega's. Je hebt dus veel vrijheid in je werk, maar staat er niet alleen voor. Je wilt niet de hele dag achter een bureau zitten, maar ook niet alleen uitvoerende zorg verlenen. Je zoekt een functie waarin je jouw kennis, zelfstandigheid en communicatieve vaardigheden kunt combineren.
