Jij zorgt ervoor dat onze cliënten zo goed mogelijk zelfstandig thuis kunnen wonen. Wat ga je doen? Als wijkverpleegkundige ben je een echte verbinder, zowel rondom de cliënt als in de wijk en binnen je team. Je indiceert en coördineert de zorg, bewaakt de kwaliteit en kijkt samen met de cliënt en zijn netwerk naar wat nodig én mogelijk is om zo lang mogelijk zelfstandig thuis te blijven wonen. Daarbij kijk je ook naar de mogelijkheden om nieuwe zorgtechnologische ontwikkelingen bij de cliënt thuis in te zetten. Als wijkverpleegkundige behoud je het overzicht over complexe zorgvragen. Daarbij is veel ruimte voor eigen initiatief om de zorg en ondersteuning voor onze cliënten te verbinden. Ook in je wijk en team ben je een coördinerend talent. Je coacht en ondersteunt je collega's, stimuleert zelfstandigheid en eigenaarschap en werkt vanzelfsprekend nauw samen met je team. Je werkt daarnaast samen met andere zorgdisciplines, huisartsen en andere zorgprofessionals.
