Je bent het aanspreekpunt voor je collega's en vervult daarin een coachende en ondersteunende rol. Je ontvangt zorgvragen en gaat op verpleegkundig adviesgesprek bij cliënten thuis. Je brengt de situatie en zorgbehoefte van de cliënt in kaart, stelt indicaties en bespreekt samen met de cliënt, familie en mantelzorgers welke zorg en ondersteuning het beste past. Samen met het team ben je verantwoordelijk voor het leveren van goede en passende zorg. Je houdt overzicht, stemt de zorg zorgvuldig af en kijkt steeds naar wat de cliënt nodig heeft om zo lang mogelijk zelfstandig en in de eigen omgeving te kunnen blijven functioneren. Daarbij kijk je verder dan alleen de directe zorgvraag. Je betrekt actief het netwerk van de cliënt en onderzoekt samen met de cliënt en betrokkenen welke ondersteuning vanuit het eigen netwerk mogelijk is en waar professionele zorg nodig is. Een goede samenwerking met huisartsen en andere partners binnen de zorgketen en de regio is daarbij belangrijk. Je weet de juiste partijen op het juiste moment te betrekken en zorgt ervoor dat de verschillende vormen van zorg en ondersteuning goed op elkaar aansluiten. Met jouw verpleegkundige expertise, helikopterview en verbindende rol draag je bij aan kwalitatief goede zorg én aan een sterk team. Je komt terecht in een enthousiast en betrokken team dat voornamelijk ouderen in Epe ondersteunt. Je krijgt veel zelfstandigheid en verantwoordelijkheid in je werk, terwijl je kunt rekenen op de steun en betrokkenheid van je collega's.
