Dit zijn wij Hallo met MIJ. Sinds 2004 bieden wij zorg en ondersteuning aan mensen met uiteenlopende culturele achtergronden. Als zorgorganisatie kun je ons vinden in de gebieden Rotterdam-Rijnmond, Dordrecht-Drechtsteden en Amstelland. Onze cliënten, zo een 3000, zijn een weerspiegeling van de steden waar zij vandaan komen. Verschillende mensen met verschillende achtergronden. Een dynamisch geheel. Met jou en zo een 800 collega s zorgen we ervoor dat onze kwetsbare cliënten op hun eigen manier en zo zelfstandig mogelijk kunnen blijven meedoen in de maatschappij. Want bij ons draait het niet alleen om goede zorg, maar ook om jouw zelfstandigheid, keuzevrijheid en kansen die écht bij jou passen.

Wat ga je doen? Als Wijkverpleegkundige in de thuiszorg inventariseer je de zorgvraag van de cliënt, zodat je op basis hiervan een zorgplan kan opstellen (OMAHA). Je houdt hierbij onder andere rekening met het handelsvermogen en niveau van functioneren van de cliënt en diens omgeving. Je coördineert de zorg thuis, stemt af met de familie en overlegt met de huisarts en andere collega's in de zorg. Daarbij kijk je uiteraard niet alleen naar de kwaliteit van zorg, maar ook naar de kwaliteit van leven.

Daarnaast behoort het evalueren en het bijstellen van het zorgplan ook tot jouw takenpakket. Je bent empatisch en in staat om een luisterend oor aan de cliënt en familie/mantelzorgers te bieden, zonder verlies van professionaliteit.
