Wijkverpleegkundige – VVT – Wormer

Ben jij een wijkverpleegkundige die graag een actieve bijdrage levert aan het organiseren en coördineren van hoogwaardige zorg binnen de thuissituatie? Wil jij jouw verpleegkundige expertise inzetten binnen een organisatie waar zelfstandigheid, persoonlijke aandacht en toekomstbestendige zorg centraal staan? Voor een vooruitstrevende zorgorganisatie in Noord-Holland zoeken wij een betrokken wijkverpleegkundige die energie krijgt van het begeleiden van cliënten, het coördineren van zorgtrajecten en het realiseren van kwalitatief hoogwaardige zorg, zodat cliënten zo lang mogelijk op een veilige en prettige manier zelfstandig thuis kunnen blijven wonen.

Wat ga je doen als Wijkverpleegkundige?

Binnen deze functie vervul je een centrale rol binnen het zorgproces en fungeer je als eerste aanspreekpunt voor cliënten, familieleden en betrokken zorgprofessionals. Je werkt nauw samen met huisartsen, behandelaren en andere ketenpartners en levert een belangrijke bijdrage aan het realiseren van passende zorg, waarbij jij verantwoordelijk bent voor zowel de verpleegkundige coördinatie als het continu optimaliseren van de kwaliteit van zorgverlening binnen de wijk.

Daarnaast begeleid, coach en ondersteun je collega’s binnen het zorgteam op inhoudelijk vlak en draag je actief bij aan het verbeteren van werkprocessen, professionele ontwikkeling binnen het team en het toekomstbestendig organiseren van zorg binnen de extramurale zorgomgeving.

Wat wordt jou geboden?

Een (direct) dienstverband van 32 uur per week

Een aantrekkelijk salaris passend binnen de CAO VVT met aanvullende arbeidsvoorwaarden (salaris van min. €3.502 en max. €4.822 o.b.v. 36 uur)
