Voor diverse gemeenten in Overijssel en Drenthe zijn wij op zoek naar bevlogen professionals die inwoners ondersteunen bij het vergroten van hun zelfredzaamheid en maatschappelijke participatie. Als Wmo-consulent ben je het eerste aanspreekpunt voor inwoners met een ondersteuningsvraag. Je voert keukentafelgesprekken, analyseert de hulpvraag vanuit een brede blik en vertaalt deze naar passende oplossingen binnen de kaders van de Wet maatschappelijke ondersteuning (Wmo 2015). Je weet de balans te vinden tussen eigen kracht, het sociale netwerk, voorliggende voorzieningen en maatwerkvoorzieningen. Met jouw ervaring ben je in staat om complexe situaties te beoordelen, weloverwogen besluiten te nemen en inwoners op een professionele en mensgerichte manier te begeleiden. - Een afgeronde HBO opleiding, bijvoorbeeld in de richting van Social Work, SJD of Ergotherapie; - Minimaal 3 jaar ervaring als zelfstandig WMO consulent; - Diepgaande kennis van de Wmo 2015; - Je bent oplossingsgericht, communicatief vaardig, en flexibel inzetbaar; - Uitstekende schriftelijke vaardigheden voor het opstellen van rapportages. - Een salaris conform CAO Gemeente schaal 9/10 €3.568 en €5.624 bruto per maand (o.b.v. 36 uur); - Een functie voor 24 tot 36 uur per week; - Hybride werken; - Vakantiegeld en een eindejaarsuitkering; - Reiskostenvergoeding of lease auto; - Persoonlijke begeleiding vanuit een vaste recruiter en accountmanager en mogelijkheden tot zelfontwikkeling; - Een betekenisvolle functie waarin je dagelijks impact maakt op het leven van inwoners.
