Dit doen we in samenwerking met (naast) betrokkenen, medewerkers ervaringsdeskundigen en netwerkpartners. Met onze methodiek vitaal versterkend werken, werken we aan een betekenisvol leven voor onze cliënten, zodat zij weer mee kunnen doen in de samenleving. Onze begeleiders zijn specialisten in het doorbreken van patronen en het versterken van de eigen regie van cliënten. Hier krijg je de kans om een belangrijke rol te spelen in het leven van cliënten met psychiatrische of psychosociale problemen, en hun reis naar herstel te ondersteunen. Coördineren van in-, door en uitstroom processen en het proces rondom herindicatie. - Coördineren van het aanmeldingstraject : Je bent verantwoordelijk voor het in gang zetten van het aanmeldingsproces door het screenen van aanmeldingen, het opvragen van aanvullende informatie en de administratieve ondersteuning. - Oriënterende- en intakegesprekken : Als onderdeel van deze functie voer je oriënterende- en intakegesprekken en bewaak je de voortgang van instroom. Betreft zowel WMO, ZVW en WLZ. - Optimale instroom : Je zorgt ervoor dat de instroom van cliënten soepel verloopt, rekening houdend met de wensen van de cliënten en onze bedrijfsmatige belangen. - Wachtlijstbeheer en plaatsingen : Je beheert mede de wachtlijst...
