O diagnóstico, a avaliação, a orientação e a estimulação dos usuários são responsabilidade da equipe multiprofissional.UBS - Unidades Básicas de Saúde (UBS) são serviços que compõem a Rede de Atenção Básica de Saúde, caracterizados pelo alto nível de capilaridade e pela oferta de serviços de saúde com maior proximidade dos usuários. O serviço se dá por meio de visitas domiciliares e do acompanhamento e da supervisão em atividades cotidianas, como idas ao mercado e passeios em parques, praças e outros locais de convivência.Responsabilidades e atribuiçõesRealizar avaliação, propor e executar manejos e adaptações que favoreçam a reabilitação física, mental, sensorial e psicológica, com finalidade de restaurar e ampliar as capacidades funcionais e atividades de vida diária do usuário, incluindo ações no domicilio. Realizar diagnóstico situacional do sujeito e/ou núcleo familiar, grupos ou território, considerando fatores de risco e proteção das condições sociais e de saúde, com o objetivo de propor estratégias de intervenção, baseadas na promoção, prevenção e recuperação em todos os ciclos de vida.
