What You'll Do Evaluate patients throughout Southern and Southwest Minnesota in their homes. Develop individualized treatment plans that promote mobility, strength, balance, and independence. Recommend home safety modifications and adaptive equipment. Educate patients and caregivers to support long-term success. Collaborate with physicians, nurses, occupational therapists, speech therapists, and the interdisciplinary care team. Complete timely, accurate documentation and monitor patient progress. What We're Looking For Active Minnesota Physical Therapist (PT) license. Excellent clinical judgment and communication skills. Passion for delivering high-quality care in community settings.
