Your Responsibilities

As a Home Care LPN, you will provide skilled nursing services under appropriate clinical direction and in accordance with Maryland nursing regulations and individualized patient care plans.

Responsibilities may include:

  • Provide direct nursing care to patients with acute, chronic, or complex medical needs.
  • Administer prescribed medications and treatments within the LPN scope of practice.
  • Monitor vital signs, assess and document patient responses, and report significant changes in condition.
  • Perform wound care and dressing changes as ordered and within demonstrated competency.
  • Assist with enteral feeding and related care when authorized and appropriately trained.
  • Provide catheter care and other nursing procedures within the authorized scope of practice.
  • Support patients with mobility limitations, chronic conditions, and post-hospitalization needs.
  • Follow physician or authorized prescriber orders and established care plans.
  • Maintain accurate, timely clinical documentation.
  • Communicate with supervising RNs, physicians, patients, and family members.
  • Educate patients and caregivers regarding prescribed care routines within the LPN scope of practice.
  • Follow infection prevention, patient safety, and agency policies.

Qualifications

Required:

  • Current, active Maryland Licensed Practical Nurse (LPN) license or valid multistate nursing license authorizing practice in Maryland.
  • Graduate of an accredited or state-approved practical nursing program.
  • Current CPR/BLS certification.
  • Strong clinical judgment, communication, and documentation skills.
  • Reliable transportation to patient assignments.
  • Ability to provide safe, compassionate, patient-centered nursing care.
  • Ability to complete required background screening, orientation, and competency assessments.

Preferred:

  • At least one year of nursing experience.
  • Previous home health, private duty nursing, rehabilitation, long-term care, or hospital experience.
  • Experience with wound care, medication administration, enteral feeding, or chronic disease management.
  • Familiarity with electronic clinical documentation.
  • Availability for evening or weekend assignments.
