What You'll Do

As a Home Health PT, you will deliver personalized therapy that helps patients move safely and confidently at home:

  • Evaluate function, mobility, strength, balance, safety, and equipment needs in the home
  • Help develop the plan of care with measurable short- and long-term goals
  • Provide therapeutic exercise, gait and transfer training, and other interventions per the plan of care
  • Reassess at each visit, complete re-evaluations, and obtain revised orders when indicated
  • Educate patients, families, and caregivers on exercises, fall prevention, and safe mobility
  • Supervise physical therapist assistants as assigned
  • Document treatment, response, and progress accurately and on time, and support discharge planning

What You Bring

  • Graduate of a CAPTE-accredited physical therapy program [or other qualifying pathway] with a current Pennsylvania PT license
  • Two years of physical therapy experience relevant to the patient population within the past five years preferred
  • Home health experience preferred
  • Current CPR certification
  • Valid driver's license, auto insurance, and reliable transportation
  • Strong clinical judgment, communication, organization, and documentation skills
