Das Gesundheitszentrum Dielsdorf ist ein modernes geriatrisches Kompetenzzentrum mit rund 500 Mitarbeitenden aus 35 Nationen. Unser Angebot reicht vom Beratungsangebot für ältere Menschen und ihre Angehörigen über die allgemeine und spezialisierte Langzeitpflege bis hin zur Pflege und Betreuung zu Hause. Als grösste öffentliche Spitex im Bezirk Dielsdorf erbringen wir ambulante Leistungen für unsere neun Vertragsgemeinden. Mit unserem umfassenden Pflegeangebot ermöglichen wir es Menschen, trotz gesundheitlicher Einschränkungen so lange wie möglich in ihrem gewohnten Zuhause zu bleiben. Zukünftig ist geplant, eine Nachtspitex zu etablieren.

Dipl. Pflegefachperson HF Spitex, Fachrichtung Psychiatrie

per sofort oder nach Vereinbarung

Ihre Aufgaben

  • Pflege, Betreuung und Beratung unserer Klientinnen und Klienten zu Hause
  • Einteilung und Gestaltung Ihrer Arbeit tagsüber von Montag bis Freitag
  • Verantwortung für die Durchführung von Abklärungen mit Inter RAI\-CMH, die psychosoziale Pflege und Betreuung von chronisch und psychisch kranken Menschen sowie das Führen von Gesprächen und recoveryorientierte Begleitung von therapeutischen Prozessen
  • Intensiver Austausch, auch über die Grenzen des Gesundheitszentrums Dielsdorf hinaus, mit Angehörigen, Sozialarbeiter:innen, der KESB, Fachstellen, Ärztinnen und Ärzten etc.
  • Aktive Mitwirkung bei der Aufbauarbeit und Weiterentwicklung der Psychiatrie\-Spitex

Ihr Profil

  • Abgeschlossene Ausbildung als Dipl. Pflegefachperson DN II/HF/FH
  • Mehrjährige Berufserfahrung im Bereich der Psychiatrie
  • Hohe Sozial\-, Methoden\- und Fachkompetenz
  • Sehr gute Deutschkenntnisse in Wort und Schrift
  • Zeitgemässe IT\-Kenntnisse (MS Office, Perigon)
  • Führerausweis Kat. B, eigenes Auto erwünscht
  • Selbstständige und verantwortungsbewusste Arbeitsweise
