Oktober 2026 suchen wir eine/n Fachperson Beratung und Aufnahme Die careköniz AG ist im Gesundheits\- und Pflegebereich tätig und bietet Dienstleistungen für ältere sowie pflege\- und hilfsbedürftige Menschen an. Zu careköniz gehören die SPITEX Region Köniz mit rund 800 Kundinnen und Kunden und das Alterszentrum Lilienweg mit 138 Plätzen. Am 1\. Januar 2025 ist careköniz aus dem Zusammenschluss von SPITEX Region Köniz und logisplus entstanden und beschäftigt heute rund 370 Mitarbeitende. Werde Teil unseres engagierten Teams und gestalte die Zukunft der Pflege aktiv mit! Das sind deine Aufgaben: Du führst professionelle Abklärungs\- und Beratungsgespräche mit den verschiedenen Anspruchsgruppen. Du informierst über die Tarife und die Grundlagen der Finanzierung des Heimaufenthaltes. Du planst den Heimeintritt und berätst und unterstützt dabei alle Beteiligten fachkundig. Du organisierst und koordinierst interne und externe Verlegungen oder Austritte nach Hause. Du informierst die Pflege über die erhobenen Assessment\-Informationen. Du arbeitest interdisziplinär mit interne und externen Fachpersonen zusammen und stellst einen professionellen Informationsaustausch sicher. Du unterstützt Angehörige beim Ausfüllen von Anträgen für die Hilflosenentschädigung. Wir wünschen uns: Du verfügst über eine Pflege\-Diplomausbildung sowie entsprechende Berufserfahrung in der Langzeitpflege, von Vorteil auch in der Akutbereich. Mit deiner Empathie und lösungsorientierten Arbeitsweise gestaltest du tragfähige Beziehungen. Du erkennst rasch die Problemfelder der Anspruchsgruppen und nutzt bei der Bedarfsabklärung dein breites Wissen und deine umfassenden Fachkenntnisse. Du verfügst über sehr gute kommunikative Fähigkeiten, sprichst und verstehst sehr gut Deutsch, ösisch\- und Englischkenntnisse sind von Vorteil. Du bist eine teamfähige und verantwortungsbewusste Persönlichkeit, die auch gerne selbständig arbeitet. Du besitzt ein Flair für administrative Tätigkeiten und bist versiert im Umgang mit Softwareprogrammen.
