Dezember 2026 oder nach Vereinbarung eine engagierte Persönlichkeit als Mitarbeiter: in Fallführung Demenz 50 – 70% / Betreuung – keine Pflege Mitarbeiter: in Fallführung Demenz 50 – 70% / Betreuung – keine Pflege Gemeinsam mit der Fachverantwortung Demenz bauen Sie unsere neue Fallführung auf. Sie begleiten Klientinnen und Klienten sowie deren Angehörige, unterstützen unsere Mitarbeitenden fachlich und gestalten das neue Angebot aktiv mit. Ihre Aufgaben Fallführung und Koordination der Betreuung von Klientinnen und Klienten mit Demenz Erst\- und Bedarfsabklärungen im häuslichen Umfeld sowie laufende Anpassung der Betreuung Fachliche Begleitung und Beratung der eingesetzten Mitarbeitenden bei anspruchsvollen Betreuungssituationen Beratung und Einbezug von Angehörigen und Bezugspersonen Zusammenarbeit mit der Fachverantwortung Demenz und externen Fachstellen sowie Mitwirkung bei Fallbesprechungen Mitgestaltung von Abläufen, Arbeitsinstrumenten und Standards für das neue Demenzangebot Ihr Profil Abgeschlossene Ausbildung im Bereich Betreuung oder Gesundheit, z. B. FaBe EFZ, FaGe EFZ oder eine vergleichbare Qualifikation Mehrjährige Erfahrung in der Begleitung und Betreuung von Menschen mit Demenz Weiterbildung im Bereich Demenz oder Bereitschaft, sich gezielt weiterzubilden Sicherheit und Einfühlungsvermögen in anspruchsvollen Demenzsituationen Fähigkeit, Betreuungssituationen fachlich einzuschätzen und Mitarbeitende kompetent zu beraten Selbständige, strukturierte und kommunikative Persönlichkeit Führerausweis Kat.
