The posting

Voor onze medische vakgroep zijn wij op zoek naar een Praktijkverpleegkundige. De praktijkverpleegkundige maakt deel uit van het behandelteam voor cliënten met chronische aandoeningen zoals diabetes mellitus, astma/COPD en cardiovasculaire problematiek.

Pak je ruimte

Als Praktijkverpleegkundige in de ouderenzorg zijn jouw werkzaamheden vooral gericht op diabetes-, COPD- en CVRM-zorg voor cliënten met een verpleeghuisindicatie of die zijn opgenomen op de revalidatieafdeling. Vanaf de dag van opname volg je de client met een van deze chronische aandoeningen om zo bij te dragen aan een goede kwaliteit van leven. Periodiek heb je contact met de cliënt en neemt, indien nodig, verpleegkundige beslissingen. Hierbij staat centraal dat de cliënt zoveel mogelijk eigen regie behoudt over zijn of haar leven.

Je werkt zelfstandig, maar ook veel samen. Dit doe je met en onder verantwoordelijkheid van de Specialist Ouderengeneeskunde, samen met de regiebehandelaren.

Daarnaast zet jij je in om kwaliteit van zorg hoog te houden. Dit doe je door de NHG-standaarden, richtlijnen en protocollen te beoordelen, vertalen en implementeren in de organisatie. Je hebt een actieve rol bij verschillende werkoverleggen, verzorgt daarbij ook klinische lessen en werkt aan je eigen professionele ontwikkeling door het volgen van diverse scholingen.
