We're seeking a passionate and proactive AOD Navigator to join our newly funded Tokoko Integrated Recovery Pathways Program in the Wide Bay region. This exciting opportunity will focus on strengthening recovery outcomes for people accessing residential alcohol and other drug treatment across Bundaberg and Hervey Bay. Working across pre-admission, treatment and post-treatment pathways, you'll help improve service coordination, strengthen referral networks and support people to access the right support at the right time. The role will also contribute to building sustainable partnerships and pathways that continue to benefit the community beyond the life of the program. About the Role As the AOD Navigator, you will play a key role in enhancing the journey into, through and beyond residential rehabilitation. Working collaboratively with clients, referrers, community organisations and service providers, you will support treatment readiness, improve access to services and strengthen coordination across the local service system. A significant focus of the role is building and maintaining referral pathways, developing collaborative relationships and improving connections between alcohol and other drug services, health providers, housing services and community supports. Key Responsibilities
Support individuals to access alcohol and other drug treatment, health, housing and community support services. Provide care coordination, service navigation and treatment readiness support. Develop and strengthen referral pathways across the Wide Bay region. Build and maintain collaborative relationships with service providers, community organisations and key stakeholders. Facilitate warm referrals and improve continuity of care across treatment and recovery pathways. Contribute to service mapping, pathway development and service integration activities. Support post-treatment engagement and connection to ongoing recovery supports. Promote recovery-oriented, strengths-based and trauma-informed practice. Maintain accurate records, reporting and stakeholder information. Contribute to continuous improvement and program evaluation activities.
This role is based in Bundaberg and involves regular travel throughout the Wide Bay region. What We're Looking For To be successful in this role, you will have:
Qualifications in Community Services, Human Services, Social Work, Behavioural Science, Psychology, Counselling or a related discipline. Experience working within the Alcohol and Other Drug, Mental Health or broader community services sector. Experience in care coordination, service navigation, case management or community support. Demonstrated ability to build effective relationships with service providers, community organisations and stakeholders. Strong understanding of recovery-oriented, trauma-informed and person-centred practice. Knowledge of alcohol and other drug treatment, recovery and harm minimisation principles. Knowledge of local health, housing and community service systems. Excellent communication, stakeholder engagement and relationship-building skills. Ability to work autonomously while contributing positively within a multidisciplinary team. Current Australian Driver Licence.
Experience within the Alcohol and Other Drug sector will be highly regarded. Why Join Lives Lived Well?
