Contract: Until March 2027 (Potential Extension) Working Hours: Office Hours, 5 Days

Job Scope:

  • Facilitate Advance Care Planning discussions with patients and families
  • Coordinate ACP documentation and ensure accessibility for healthcare teams
  • Promote ACP awareness through education, training, and community engagement
  • Monitor ACP outcomes, maintain reports, and support quality improvement initiatives
  • Collaborate with multidisciplinary teams and community partners to enhance ACP services
  • Support ACP events, projects, research, and programme development

Requirements:

  • Bachelor's Degree in Nursing, Psychology, Social Work, Counselling, or related Allied Health discipline
