Job Scope:

  • Assess patients' lifestyle, dietary habits and diabetes management needs
  • Provide diabetes education and personalised counselling
  • Educate patients on insulin injections and glucose monitoring
  • Review glucose profiles and support individualised care plans
  • Work closely with Endocrinologists and support quality improvement initiatives

Requirements:

  • Valid SNB
  • Advanced Certification in Diabetes required
  • Relevant diabetes nursing / patient education experience
