Responsibilities
- Conduct psychosocial assessments to identify patients’ social, emotional, financial and family-related needs.
- Provide counselling, emotional support and crisis intervention to patients and their families.
- Develop and implement appropriate social work intervention plans based on individual patient needs.
- Coordinate referrals and connect patients with community resources, financial assistance and relevant support services.
- Collaborate with multidisciplinary healthcare teams to support discharge planning, continuity of care and complex case management.
- Maintain accurate case records, prepare reports and ensure professional, ethical and confidential social work practice.
Requirement
- Degree in Social Work or related courses
- Able to commit to Monday to Friday office hours, occasional half Saturday
- Min 2 years or relevant experience
