The posting

The Social Work Associate plays a vital operational role within WE CARE’s Sober Living Framework (SLF). Acting as a critical link between therapeutic recovery and community resources, this role is specifically structured to assist inpractical, resource-heavy case management and operational coordination tasks. By managing welfare applications, shelter coordination, and hospital detox pipelines, the Social Work Associate ensures that clients are facilitated to achieve holistic lifestyle stability while allowing the professional counselling team to maximize dedicated clinical hours for therapy.

KeyResponsibilities

1. Practical Case Management & Resource Support

  • Identify structural barriers to a client's recovery and coordinate practical interventions before or during intensive clinical therapy.
  • Assist clients in navigating and applying for financial assistances, including Social Service Office (SSO) subsidies and other welfare streams.
  • Facilitate logistical pipelines for acute interventions, such as managing admissions to local restructured hospitals or specialised units (e.g., IMH, NAMS) for medical detoxification.
  • Source and coordinate temporary housing, halfway houses, or institutional shelter arrangements for vulnerable clients in unstable living environments.

2. Intake, Triage & Administration

  • Support the triaging team by conducting initial administrative screening and eligibility verifications for new incoming community or agency referrals.
  • Assist the Clinical Supervisor in routing qualified addiction cases to the designated counselling team members.
  • Ensure strict data entry compliance and timeline adherence within the Enhanced Programme Evaluation System (EPES) and internal case management systems.

3. Client Retention & Care Coordination

  • Execute the centre’s outreach protocols to proactively re-engage and follow up with clients who miss sessions, are uncontactable, or risk dropping out of the recovery track.
  • Collaborate closely with Counsellors and peer Recovery Guides to ensure an integrated, wrap-around continuum of care for the individual client.
  • Guide transitioning clients toward recovery-supportive platforms, such as the WE CARE Drop-in Centre and peer-led support groups (e.g., Active Recovery Circles).

4. Multi-Agency & Stakeholder Liaison

  • Serve as an operational point of contact for external social service agencies, including Family Service Centres (FSCs) transferring complex cases via the sector-wide Addiction Support Service (ASS) pipeline.
  • Maintain positive working workflows with medical social workers and step-down discharge units across our 10 partner restructured hospital networks.

Requirements

  • Diploma in Social Work, Social Sciences, Psychology, or a relevant human services discipline. (Candidates with a Professional Certification in Social Work or relevant foundational training are welcome to apply).
  • Minimum 1–2 years of experience in the social service sector, community healthcare, or a casework management setting. Prior exposure to addiction recovery, mental health care, or vulnerable family support is highly advantageous.
  • Strong knowledge of Singapore's social service network, government subsidies, and community support schemes (MSF, SSO, AIC, etc.).
  • Excellent case-documentation skills and is comfortable utilizing database software (EPES or similar client data management portals).
  • Compassionate, non-judgmental, and highly resilient attitude when working with individuals recovering from substance or behavioural dependencies.
  • Ability to work effectively in a multidisciplinary team environment alongside clinical counsellors, peer leaders, and hospital networks.
  • Strict adherence to professional ethics, confidentiality protocols, and the Personal Data Protection Act (PDPA).
