Key Responsibilities
Psychosocial Assessment & Intervention
- Conduct biopsychosocial assessment, screening and risk identification.
- Develop and review individualised care and intervention plans.
- Provide counselling, psychosocial intervention and caregiver support within professional scope.
- Manage complex cases and respond to safeguarding or higher-risk concerns.
- Facilitate multidisciplinary case discussions and appropriate referrals.
Community Outreach & Engagement
- Conduct outreach, case finding and early identification activities.
- Support mental health and dementia awareness and preventive activities.
- Engage community partners to facilitate timely identification and referral.
Case Coordination & Partnerships
- Coordinate healthcare, social and community services.
- Support clients and caregivers in navigating available resources.
- Build collaborative relationships with healthcare and community partners.
Professional Practice & Programme Development
- Provide professional consultation and guidance on complex cases.
- Contribute to practice development, programme evaluation and service improvement.
- Maintain accurate case documentation and support programme reporting.
Requirements
- Graduate Diploma or Degree in Social Work recognised by SASW or Master's in Counselling recognised by SAC.
- Relevant experience in social work, counselling, mental health, dementia, healthcare or community care preferred.
- Strong biopsychosocial assessment, counselling, communication and stakeholder engagement skills.
- Able to work independently and within a multidisciplinary team.
