The posting

Employment Type: Permanent Salary: $4,500–$5,000 Working Hours: 5 Days, Office Hours

Job Scope:

  • Provide nursing care to patients in their homes
  • Conduct assessments and develop individualised care plans
  • Train and support patients' caregivers
  • Support End-of-Life care for homebound patients
  • Maintain documentation and coordinate with the care team

Requirements:

  • Valid SNB registration
  • Minimum 4 years of Medical / Geriatric nursing experience
  • Recent acute or community nursing experience required
  • Oncology / Palliative Care experience is an advantage
  • Able to support rotational after-hours tele-consult
