Job Responsibilities
- Provide holistic and compassionate nursing care to patients receiving home care and palliative care services.
- Conduct nursing assessments and monitor patients' physical and psychosocial conditions.
- Develop and implement appropriate nursing care plans based on patients' needs.
- Manage symptoms and provide appropriate palliative nursing interventions.
- Administer medications and treatments in accordance with doctors' orders and established protocols.
- Provide wound care, pain management, and other nursing procedures where required.
- Educate patients, caregivers, and family members on disease management, medication, and end-of-life care.
- Work closely with doctors, allied health professionals, caregivers, and other members of the multidisciplinary team.
- Provide emotional and psychosocial support to patients and their families.
- Maintain accurate and timely nursing documentation and patient records.
- Participate in care coordination and follow-up to ensure continuity of care.
- Adhere to professional nursing standards, infection control procedures, and organisational policies.
- Perform other duties as assigned.
Requirements
- Registered with the Singapore Nursing Board (SNB).
- Valid BCLS certification.
- Relevant palliative care experience is required.
- Relevant palliative care certification / qualification is required.
- Strong communication and interpersonal skills.
- Compassionate and patient-centred approach to care.
- Able to work independently and as part of a multidisciplinary team.
- Comfortable providing care in a home and community setting.
