Wat je gaat doen

De ouderenzorg is enorm in beweging. Het aantal ouderen neemt toe en er wordt steeds meer ziekenhuiszorg verplaatst naar thuiszorg. Als Wijkverpleegkundige in de thuiszorg kom je daarom met grote regelmaat complexe situaties tegen. Situaties die vragen om een doortastende persoonlijkheid; die vragen om een wijkverpleegkundige die een centrale en leidende rol vervult. Jij coördineert de zorg thuis, stemt af met de familie en overlegt met de huisarts en andere collega’s in de zorg. Jij bent degene die problemen signaleert én aanpakt. Daarbij kijk je uiteraard niet alleen naar de kwaliteit van zorg, maar naar de kwaliteit van leven.

Wie ben jij?

Jij bent een ervaren en betrokken verpleegkundige die moeiteloos schakelt tussen verschillende partijen en het overzicht weet te bewaren. Met jouw coachende instelling, ondernemende houding en affiniteit met de doelgroep geef je deze functie verder vorm en draag je bij aan de best mogelijke zorg.

Je brengt onder andere mee:

  • een afgeronde hbo-opleiding Verpleegkunde;
  • sterke communicatieve vaardigheden;
  • een geldige BIG-registratie en inschrijving in het Kwaliteitsregister;
  • een zelfstandige werkhouding, waarbij je ook goed samenwerkt binnen een team;
  • een rijbewijs en eigen vervoer.

Wij zorgen voor

  • een contract voor onbepaalde tijd op basis van (aantal uur) uur per week;
  • een aantrekkelijke eindejaarsuitkering van 8,33% van het bruto jaarinkomen;
  • een inschaling volgens FWG 50 volgens de cao VVT;
