Werken in de wijkteams Pijnacker-Nootdorp-Delfgauw is werken in een dorpse, groene omgeving in de nabijheid van grote steden. De 2 wijkteams werken vanuit de teampost in hartje Pijnacker. Je speelt een belangrijke rol in het ondersteunen van cliënten thuis, met oog voor hun wensen en mogelijkheden. Daarbij kun je rekenen op de kracht van een professioneel en betrokken team.

Dit ga je doen als Wijkverpleegkundige

Als Wijkverpleegkundige bied je zorg op maat in de thuissituatie, waarbij je zowel de medische zorg als het welzijn van de cliënt versterkt. Je stimuleert zelfredzaamheid, zelfregie en preventie. Je draagt zo bij aan een betere kwaliteit van leven.

Concreet betekent dat:

  • Je stelt de behoefte aan verpleegkundige zorg vast door middel van klinisch redeneren.
  • Je coördineert indiceringsprocessen en biedt passende zorg als poortwachter voor de wijkverpleging.
  • Je versterkt het zelfmanagement van cliënten en hun sociale context.
  • Je neemt regie in complexe zorgvragen en coördineert met cliënten en andere zorgprofessionals.
  • Je begeleidt en coacht teamleden en stagiaires in de zorgverlening.
  • Je signaleert verbeterpunten in zorgprocessen en implementeert innovaties.
  • Je creëert een open en veilige sfeer binnen het team en rondom het omgaan met feedback.

Wat je van ons mag verwachten

  • Salaris volgens cao VVT, FWG 50: € 3.592,96 - € 4.822,50 (o.b.v. 36 uur per week).
  • Direct een vast contract voor 28 tot 36 uur per week.
  • Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.
  • Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.
  • Toegang tot o.a. HeyCoach, budgetcoach en DigiCoach voor jouw ontwikkeling.

Wat jij meebrengt

  • Een hbo-diploma Verpleegkunde en geldige BIG-registratie.
  • Communicatieve vaardigheden en een empathische houding.
  • Je bent een echte teamspeler en hebt een proactieve werkhouding.
  • Ervaring in de ouderenzorg is een pre.

Om de veiligheid van onze cliënten en medewerkers te waarborgen, is voor alle medewerkers die bij Careyn in dienst komen een Verklaring Omtrent Gedrag (VOG) vereist. Bij gelijke geschiktheid krijgen interne sollicitanten voorrang ten opzichte van externe kandidaten.
