Build and maintain strong relationships with network partners and relevant health, social, and community service providers. Leverage PCH’s expertise in complex aging to enhance care integration, improve patient outcomes and experiences, and support primary care providers in managing high-needs patients within a coordinated, patient-centred model of care. Promote PCH programs, services, and navigation supports within the Mississauga community using a person-centred approach. Contribute to a safe, welcoming, non-judgmental, and comfortable care environment for patients/clients, families, and caregivers. Communicate effectively with patients, families, caregivers, community partners, and members of the healthcare team. Practice collaboratively with members of the healthcare team to provide care and support to patients/clients with complex health needs. Ensure practice is consistent with entry-to-practice competencies, safe practice standards, professional standards, guidelines, and applicable legislation. Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
