Jouw werkplek

In Haarlem ondersteun je zelfstandig wonende cliënten met een licht verstandelijke beperking (LVB), soms in combinatie met autisme, verslavingsproblematiek of psychiatrische problematiek. Je bezoekt cliënten thuis en begeleidt hen ook tijdens groepsmomenten op ontmoetingsplekken. Eigen regie en zelfredzaamheid staan centraal: je kijkt naar wat iemand zelf kan, welke ondersteuning vanuit het netwerk mogelijk is en waar professionele begeleiding nodig blijft.

Jouw team

Als Coördinerend Ambulant Begeleider werk je in een ambulant team van 6 collega’s. Hoewel je veel zelfstandig op pad bent, zoek je elkaar op om complexe casuïstiek te bespreken en kennis te delen. Je werkt daarnaast samen met gemeenten, ketenpartners en andere organisaties in de wijk om de ondersteuning rondom cliënten goed af te stemmen.

Jouw uitdaging

Je begeleidt cliënten thuis, digitaal en tijdens groepsmomenten. Je helpt bij het aanbrengen van structuur, stimuleert zelfredzaamheid en werkt samen met cliënten aan hun doelen. Je signaleert veranderingen in gedrag, welzijn en ondersteuningsbehoefte en kijkt steeds welke vorm van ondersteuning passend is.

Daarnaast neem je de coördinatie rondom cliënten op je. Je stelt ondersteuningsplannen op, bewaakt doelen en voortgang en bent verantwoordelijk voor een goede vastlegging in het ECD. Bij complexe casuïstiek houd je overzicht, verbind je de betrokken partijen en zorg je dat afspraken worden vertaald naar de dagelijkse ondersteuning.

Daarbij durf je zowel te ondersteunen als los te laten. Je kijkt niet alleen naar welke hulp mogelijk is, maar vooral naar wat iemand daadwerkelijk nodig heeft om verder te kunnen. Juist daarin komen liefde, lef en meesterschap samen. Je bouwt aan vertrouwen, durft verantwoordelijkheid te nemen en zet jouw professionele kennis in om cliënten zoveel mogelijk regie over hun eigen leven te geven.

Wat krijg je van ons?

Bij Philadelphia krijg je de ruimte om het beste uit jezelf en uit je werk te halen. Daarom bieden wij naast alles uit de CAO Gehandicaptenzorg ook extra’s.

  • Een contract voor onbepaalde tijd voor 28 uur per week, waarbij minder uren bespreekbaar zijn, met een salaris volgens FWG 45 (€ 3.113 tot € 4.347 bruto per maand o.b.v. 36 uur).
  • Pensioen, 8% vakantiegeld en 8,33% eindejaarsuitkering. Vakantiegeld en eindejaarsuitkering kun je ook maandelijks laten uitbetalen.
  • Veel zelfstandigheid en eigen regie over je caseload, met afwisseling tussen begeleiding thuis, digitale ondersteuning en ontmoetingsplekken.
  • Voor je werkzaamheden krijg je een laptop en ondersteuning bij het inrichten van je thuiswerkplek. Ook kun je gebruikmaken van onder andere coaching, fitness en mindfulness.
  • Ruimte om je vakmanschap verder te ontwikkelen via het Philadelphia Leerhuis en om jouw kennis en ideeën in te brengen in projecten en de verdere ontwikkeling van ambulante begeleiding.

Wie ben jij?

Je kunt zelfstandig werken, maar zoekt actief de samenwerking wanneer een ondersteuningsvraag daarom vraagt. Je houdt overzicht in complexe situaties en weet professionele nabijheid te combineren met duidelijke grenzen en eigen regie voor de cliënt.

Wat je verder meebrengt:

  • Een afgeronde relevante hbo-opleiding in Zorg & Welzijn.
  • Een aantal jaar ervaring als Coördinerend Begeleider en ervaring met de coördinerende verantwoordelijkheden die daarbij horen.
  • Goede communicatieve en administratieve vaardigheden en ervaring met het werken in een ECD.
  • Je beschikt over een auto, zodat je zelfstandig cliënten in de regio Haarlem kunt bezoeken.
