Zie jij wat een ander nodig heeft, ook als het niet wordt uitgesproken? Dan ben jij precies de Coördinerend Begeleider LVB die wij zoeken op de Albertus Perkstraat in Hilversum. Jij krijgt energie van verantwoordelijkheid, overzicht creëren en echt contact maken met bewoners en collega’s. Je bent iemand die rust en structuur kan bieden, terwijl je tegelijkertijd ruimte laat voor persoonlijke groei en zelfstandigheid van de bewoners.

Op de Albertus Perkstraat wonen 31 volwassenen met een licht verstandelijke beperking (LVB). Ze leven zelfstandig in hun eigen appartement, maar hebben dagelijks begeleiding nodig die aansluit bij hun unieke behoeften:

  • De één zoekt nabijheid en structuur, iemand die meedenkt en ondersteunt bij dagelijkse routines;
  • De ander heeft een duwtje in de rug nodig bij planning, administratie of invulling van de dag.

Jij bent de stabiele factor die overzicht bewaart, rust uitstraalt en aanvoelt wat er nodig is ook als dit nog niet wordt uitgesproken. Door jouw aanwezigheid voelen bewoners zich veilig, gehoord en ondersteund.

Je werkt in een warm en betrokken team van ongeveer 15 collega’s, in een sfeer die vertrouwd en nuchter is. Hier is ruimte voor humor, initiatief en jouw ideeën. Het team werkt volgens de principes van regelarme zorg. Dat betekent dat jij, samen met je collega’s, actief meedenkt en mede de regie hebt over de invulling van de begeleiding. Je krijgt hierin de ruimte om professionele afwegingen te maken, altijd met oog voor de bewoners en hun behoeften.. De locatie ligt centraal in Hilversum en is uitstekend bereikbaar met fiets, auto en OV.

Wat ga je doen?

Als coördinerend begeleider ben je de schakel tussen dagelijkse praktijk, team en organisatie. Je zorgt voor afstemming, continuïteit én nabijheid in de begeleiding. Je:

  • coördineert de zorg rondom een vaste groep bewoners en stelt samen doelen op;
  • werkt nauw samen met gedragsdeskundigen en familie;
  • bewaakt de voortgang van begeleidingsplannen en pakt signalen op;
  • begeleidt collega’s in de uitvoering, stimuleert reflectie en samenwerking;
  • draait mee in de onregelmatige diensten, inclusief slaapdiensten.

Wat neem je mee?

  • Een mbo-4 diploma in zorg of welzijn (bijv. MMZ, Persoonlijk Begeleider, SPW, of vergelijkbaar);
  • Ervaring in het werken met mensen met een verstandelijke beperking (liefst LVB);
  • Coachende vaardigheden en het vermogen om het grotere plaatje te zien;
  • Flexibiliteit, rust en overzicht. Ook in uitdagende situaties;
  • Een energieke houding, teamgevoel en lef om initiatief te nemen.

Wat bieden wij jou?

  • Een contract voor onbepaalde tijd voor 28-32 uur per week;
  • Salaris volgens CAO Gehandicaptenzorg, FWG 40: (€ 2.893 - € 3.902 bij 36 uur);

  • Extra’s zoals een fietsregeling, laptopregeling en korting op uitjes;
  • Ondersteuning via het opvangteam van Philadelphia als werk of leven even veel vraagt.
