Anfieldroad in Amsterdam Oost is geen standaard woonlocatie. Het is een plek met karakter, geschiedenis en mensen die ertoe doen. Hier wonen 26 ouder wordende mensen met een (licht) verstandelijke beperking. Sommigen zijn zelfstandig en eigenwijs, anderen hebben meer nabijheid en structuur nodig. Wat zij gemeen hebben? De behoefte aan aandacht, stabiliteit en vertrouwde gezichten.

Onze locatie is volop in beweging. We bouwen aan een stevig, betrokken team dat weet wat goede ouderenzorg binnen de gehandicaptenzorg betekent vandaag en morgen. En dat begint bij sterke coördinerend begeleiders zoals jij mensen met overzicht, lef en een warm hart.

Jij brengt rust waar het onrustig is. Richting waar het schuurt. En nieuwe energie waar het vastloopt. Niet vanachter een bureau, maar midden in het leven van bewoners en collega’s.

Wat ga je doen?

Als coördinerend begeleider ben jij de verbindende schakel tussen bewoners, collega’s, gedragsdeskundigen en externe partners. Je combineert inhoudelijke expertise in de zorg met coördinerende taken en teamontwikkeling. Concreet betekent dat:

  • De dagelijkse begeleiding en lichte zorg (ADL, verbale aansturing, medicatie) wordt door jou gecoördineerd;
  • Het ondersteuningsplan wordt vertaald naar de praktijk, waarbij het team deze werkwijze begrijpt, toepast en uitdraagt;
  • Bewoners worden methodisch begeleid, onder andere volgens belevingsgerichte zorg, en ondersteund bij een passende daginvulling;
  • Collega’s worden op de werkvloer gecoacht, waarbij reflectie wordt gestimuleerd en methodisch werken wordt versterkt;
  • Kansen worden gesignaleerd, initiatief genomen en actief meegewerkt aan toekomstgerichte ouderenzorg;
  • Zichtbaar en nabij aanwezig zijn wordt gecombineerd met overzicht bewaren, zodat contact behouden blijft zonder het grotere geheel uit het oog te verliezen.

Wie ben jij?

Wij zoeken geen meedraaiende kracht, maar een stevige kartrekker. Iemand die naast het begeleiden van bewoners ook actief bouwt aan samenwerking, eigenaarschap stimuleert en energie brengt in de dag. Je ziet het grotere geheel, durft besluiten te nemen en weet mensen mee te nemen. Juist in een omgeving die in ontwikkeling is.

Wat neem jij mee?

  • Een afgeronde zorgopleiding op mbo‑4 of hbo‑niveau (MMZ, SPH, Social Work e.d.);
  • Ervaring met LVB-doelgroep (ouderen) in de gehandicaptenzorg of ouderenzorg;
  • Coachende vaardigheden en het vermogen om methodisch te werken en dit over te dragen;
  • Helikopterview en daadkracht, waarbij duidelijk wordt wat nodig is en dit benoemd wordt;
  • Affiniteit met community building, waardoor verbinding ontstaat tussen mensen binnen en buiten de locatie;
  • Bereidheid tot werken in dag-, avond- en slaapdiensten.

Wat bieden wij jou?

  • Een vast contract voor 28 tot 32 uur per week;
  • Salaris volgens FWG 40: € 2.951 - € 3980 bruto p/m op basis van 36 uur, CAO Gehandicaptenzorg);
  • Scholing, coaching en ontwikkeling (o.a. teamontwikkeling, coachend leiderschap);
  • Fijne locatie met 18 collega’s, interne dagbesteding en gratis parkeergelegenheid;
  • Meebouwen aan een team in ontwikkeling met veel ruimte voor jouw ideeën;
  • Ruimte voor meeloopmomenten, vaste gespreksdagen en korte lijnen met je manager.
