Wat ga je doen?

Voor een huisartsenpraktijk in Den Haag zoeken wij een enthousiaste Praktijkondersteuner Somatiek voor 16 uur per week. Een praktijk waar je het verschil kunt maken!

Je gaat aan de slag in een huisartsenpraktijk met ongeveer 4.200 patiënten. Je krijgt veel ruimte om zelfstandig te werken en een belangrijke rol te spelen in de organisatie en verdere ontwikkeling van de chronische zorg. Als POH-S begeleid je patiënten met diabetes mellitus (DM), cardiovasculair risicomanagement (CVRM), astma en COPD. Daarnaast voer je longfunctieonderzoek uit en ben je de kartrekker van de verschillende zorgprogramma's. Je werkt nauw samen met de huisartsen en kunt overleggen met een collega-POH die zich volledig richt op de ouderenzorg. Zo heb je veel zelfstandigheid, maar kun je ook gemakkelijk sparren met collega's. De praktijk is aangesloten bij zorggroep Hadoks en werkt met MicroHIS en VIPLive.

Wat maakt deze praktijk aantrekkelijk? - Een zelfstandige functie met veel ruimte voor eigen initiatief. - De mogelijkheid om de chronische zorg verder te organiseren en ontwikkelen. - Een overzichtelijke praktijk met korte lijnen en betrokken collega's. - Een afwisselende functie waarin je zowel patiëntenzorg verleent als een coördinerende rol vervult. - Een dienstverband van 16 uur per week, waarbij de werkdagen in overleg worden afgestemd.

Wanneer is dit iets voor jou?

We zoeken een collega die: - Een afgeronde opleiding tot POH-S heeft en beschikt over relevante werkervaring. - Zelfstandig spreekuren kan voeren voor DM, CVRM, astma en COPD. - Ervaring heeft met longfunctieonderzoek en bij voorkeur Caspir-gecertificeerd is. - Initiatief neemt, overzicht houdt en graag meedenkt over verbeteringen. - Zelfstandig werkt, maar ook gemakkelijk de samenwerking met collega's opzoekt. - Het leuk vindt om een voortrekkersrol te vervullen binnen de chronische zorg.

Voor deze functie zoeken we een gediplomeerde POH-S die zelfstandig aan de slag kan. Er is ruimte om je wegwijs te maken binnen de praktijk, maar niet voor het begeleiden van iemand die nog in opleiding is.

  • Veel aandacht voor scholing en persoonlijke ontwikkeling. In 2025 was er ruim één week aan leertijd per praktijkondersteuner. Denk hierbij aan coaching, intervisie, kennisdeling en training & opleiding;
  • 144 vakantie-uren en 57 PLB-uren bij een fulltime (36-uur) dienstverband. PLB staat voor Persoonlijk LevensfaseBudget.
