Vi søger en ny kollega til Hjemme- og sygeplejen i Store Heddinge. Stillingen er som udgangspunkt på 37 timer om ugen fortrinsvis i dagvagt. Vi er dog fleksible, så timetal kan tilpasses dine ønsker. Der er også mulighed for blandede dag- og aftenvagter.

Hvem er vi? Vi er opdelt i to mindre teams, som består af sygeplejersker, social- og sundhedsassistenter samt social- og sundhedshjælpere. Vi lægger stor vægt på at skabe målrettede, individuelle borgerforløb af høj kvalitet. Hos os får du en rummelig arbejdsplads med højt til loftet. Vi hjælper hinanden i hverdagen, prioriterer faglig refleksion og sparrer tæt med hinanden. Vores ledelsesteam, fagkoordinatorer og planlæggere sikrer et stærkt læringsmiljø og en tryg hverdag.

Dine opgaver og hverdag:

  • Varetage komplekse og udfordrende opgaver i borgernes egne hjem.
  • Sikre sammenhængende og koordinerede forløb for den enkelte borger.
  • Indgå i et tæt og tværfagligt samarbejde med udekørende sygeplejersker og andre partnere.
  • Fokusere på den gode relation og kerneopgaven i tæt kontakt med borgeren.

  • Du tager ansvar og kender dine egne faglige kompetencer.
