This position includes weekend work.

Classification for this position will be Allied Health Grade 2 / relevant nursing classification, dependent on the successful applicant's professional background.

Duties

  • Provide high-quality, evidence-based clinical assessment, intervention, education and management.
  • Undertake phone-based clinical assessments to identify clients' health and support needs following discharge.
  • Use remote monitoring technology to triage clients and determine appropriate care pathways.
  • Develop collaborative short-term care plans and coordinate services to address immediate health and wellbeing needs.
  • Monitor client progress and health outcomes, supporting appropriate transition and discharge from the program.
  • Work collaboratively with hospital, community and external service providers to facilitate continuity of care.
  • Contribute to quality improvement initiatives, team processes and ongoing service development.

Your Contribution

  • Nursing or Allied Health qualification with current registration with the Australian Health Practitioner Regulation Agency (AHPRA), where applicable.
  • Minimum two years' experience in a relevant acute and/or community healthcare setting.
  • Strong clinical assessment and management skills, with experience in discharge planning and coordination of care.
  • Experience supporting clients with multiple health conditions, psychosocial concerns and complex care needs.
  • Demonstrated ability to work collaboratively with clients, families and internal and external service providers.
  • Ability and willingness to work within a phone-based and virtually enabled model of care, including weekend work.
  • Excellent organisational, care coordination and time management skills.
  • Current National Criminal History Check, or willingness to obtain one.

What We Offer

  • A focus on wellbeing initiatives, with regular events and programs.
  • Confidential, solutions-focused employee counselling.
  • Access to Fitness Passport, providing a wide choice of fitness facilities.
  • Salary packaging options to increase your take-home pay.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
