Social Services Case Manager
About the Role Are you a social worker who thrives on connecting patients to the resources that change their lives — housing, food, insurance, safety, treatment? Thundermist Health Center is looking for a Social Services Case Manager to join our Behavioral Health & Social Work Services team. You’ll provide case management, financial resources counseling, crisis intervention, and supportive counseling to patients who face real barriers to getting and staying healthy. You’ll work as part of an integrated clinical team alongside medical providers, behavioral health clinicians, and community partners — advocating for patients and meeting them where they are. If you’re passionate about health equity, social determinants of health, and serving underserved communities, this is your role. What You’ll Do Case Management & Patient Advocacy
Provide psychosocial assessment and determine the need for case management services Develop individualized treatment plans with patients, setting goals and objectives in partnership with the patient and care team Provide ongoing case management, supportive counseling, financial resources counseling, and crisis intervention Advocate for patients in reducing barriers to healthcare, including housing, food security, transportation, insurance access, and immigration-related concerns Make referrals for mental health and substance use counseling, DCYF, HIV/AIDS case management, Health Care for the Homeless, Family Planning, and other services Provide prenatal assessment, perinatal case management, counseling, and referrals to community agencies supporting pregnant and parenting patients Accept referrals from medical providers, behavioral health clinicians, and external agencies as a member of the integrated clinical team
Community Engagement & Program Development
Build and maintain relationships with community agencies, social service partners, and public health entities serving Thundermist patients Develop, implement, and coordinate psychoeducational programs targeting public health prevention initiatives Participate in community committees, coalitions, and initiatives as appropriate
Documentation & Compliance
Document all patient interactions clearly and concisely in the EHR and social service record Ensure compliance with HIPAA, 42 CFR Part 2 (SUD confidentiality), DCYF reporting requirements, and applicable regulations Maintain strict confidentiality of patient medical records, financial information, and documentation from outside agencies Participate in clinical supervision, chart reviews, and quality improvement initiatives
What We’re Looking For You’re a case manager who sees the whole person — not just the diagnosis, but the housing situation, the insurance gap, the childcare barrier, the immigration concern. You navigate complex systems with patience and persistence, and you know the Rhode Island resource landscape well enough to connect patients to what they actually need.
Bachelor’s degree in Social Work or a related field Minimum 1 year of social work case management experience with high-risk families and individuals Working knowledge of Rhode Island community resources and public benefits systems, including DCYF Working knowledge of HIPAA and 42 CFR Part 2 (SUD confidentiality) Strong interpersonal skills and ability to work collaboratively with patients, families, providers, and community agencies Exceptional oral and written communication skills
Preferred:
Prior experience in an FQHC, community health center, or safety-net healthcare setting Experience with HIV/AIDS case management, Ryan White programs, Health Care for the Homeless, or Family Planning Experience with perinatal case management and prenatal assessment Bilingual (Spanish/English or Portuguese/English) strongly preferred Experience with motivational interviewing, trauma-informed care, and harm reduction approaches Case Management certification (CCM or ACM) or Rhode Island Social Work licensure (LSW/LICSW)
Schedule & Location Full-time (40 hours/week), benefit-eligible, based on-site at an assigned Thundermist location. Mobility required to meet with patients throughout the health center, with occasional home visits as scheduled. Some evening sessions required to support patient access, and flexibility to respond to patient crises as needed. Occasional travel between Thundermist sites and to community agencies and partner organizations. Benefits at Thundermist Health Center Full-Time Employees Working 40+ Hours/Week
