The posting

In de regio Zuid-Limburg werken begeleiders met elkaar samen in wijkteams. In deze samenwerking bundelen we onze krachten om passende ondersteuning te bieden bij uiteenlopende cliëntvragen, afgestemd op de mogelijkheden binnen de wijk.

We werken aan het persoonlijk herstel van de cliënt en het versterken van hun plek in de maatschappij. Denk aan het uitbreiden van het sociale netwerk, het vinden van passende dagbesteding of het stimuleren van deelname aan werk en opleiding.

Psychiatrische, psychische of verslavingsproblemen kunnen een grote impact hebben op iemands leven. Wij helpen mensen klachten te verhelpen of te verminderen. In onze visie biedt het alledaagse leven veel herstelkracht. Die kracht willen we beter benutten in de ggz. De kans op herstel is het grootst als mensen direct de juiste zorg krijgen: thuis in de eigen omgeving en toegespitst op wat voor hen werkt met de mensen om hen heen: professionals, naasten en familie.

Ons team heeft je hart nodig

Als ambulant begeleider ben je bezig met het op basis van leef-/zorgplannen of ondersteuningsplannen, psychosociaal en/of praktisch begeleiden en ondersteunen van toegewezen psychische kwetsbare burgers. Daarbij ben je gericht op het stimuleren van de zelfredzaamheid bij het wonen en functioneren in de samenleving, het onderhouden van sociale contacten, het invullen van dagbesteding en ontspanning en het daarbij bieden van structuur. Je levert een bijdrage aan de kwaliteit van de begeleiding en ondersteuning van deze doelgroep. Voor deze vacature zoeken we een begeleider voor het team Maastricht/Heuvelland.

  • Een zelfstandige collega met een passende HBO- óf mbo zorg diploma;
  • Oplettendheid gedurende de uitvoering van de begeleiding en ondersteuning en het tijdig kunnen signaleren van gedragsverandering;
  • Sociale vaardigheden voor het intensief begeleiden en ondersteunen van de psychisch kwetsbare burgers en diens steunsysteem waarbij sprake kan zijn van intensieve contacten, gericht op persoonlijke groei;
  • Je werkt doelgericht en weet dit te vertalen naar een helder en passend ondersteuningsplan. Dit is een essentieel onderdeel van jouw rol;
  • Je beschikt over kennis van en inzicht in de sociale kaart;
  • Om in aanmerking te komen voor deze functie is een positieve Verklaring Omtrent Gedrag (VOG) vereist;
  • Het natrekken van referenties en het controleren van diploma’s maken onderdeel uit van de sollicitatieprocedure.

  • We bieden je een salaris conform CAO GGZ FWG 45. Het salaris bedraagt minimaal €3180,- en maximaal €4273,-;
  • Hoeveel uur wil je werken? Het aantal uren van je aanstelling is bespreekbaar tussen de 24 en 28 uur;
  • Je bezoekt je cliënten tussen 09:00 en 17:00 uur en je bent de weekenden vrij.
