The posting

Le Centre de Soins Infirmiers de Douai, membre du GCSMS VIVAT, est en cours de transformation en Centre de Santé Polyvalent.

  • Deux nouveaux sites sont en cours de construction : Flines-lez-Râches 59148 (déménagement du CSI de Marchiennes) et Aniche 59580. Ces communes figurent parmi les territoires à plus faible densité médicale de la région. Des milliers de personnes n'y ont pas de médecin traitant.

Ce projet est porté par VIVAT, groupement qui accompagne plusieurs milliers de familles dans les Hauts-de-France à travers ses services d'aide à domicile et ses centres de santé infirmiers. Le modèle est construit sur une conviction : soigner correctement, c'est d'abord soigner dans de bonnes conditions.

  • Missions :

Cliniques :

→ Consultations médicales au cabinet et, selon projet, visites à domicile

→ Prise en charge des pathologies aiguës et chroniques, polypathologies, soins palliatifs

→ Suivi des patients en perte d'autonomie, en lien avec les infirmières et le SAD VIVAT

→ Prescription, orientation, coordination du parcours de soins

Coordination et vie du centre :

→ Participation aux réunions de coordination pluriprofessionnelle

→ Alimentation du Dossier Médical Partagé (DMP)

→ Contribution à l'élaboration de protocoles de soins coordonnés

→ Participation à la Permanence Des Soins Ambulatoires (PDSA) selon zonage

Profil recherché :

Formation et autorisation d'exercice

→ Diplôme d'État de Docteur en Médecine

→ Inscription à jour à l'Ordre des Médecins en France ou possibilité d'exercer en France

→ DU ou formation complémentaire en gériatrie, soins palliatifs ou médecine de parcours : apprécié

→ Capacité à travailler en autonomie tout en s'inscrivant dans un collectif soignant

→ Sens de la continuité : connaître les patients dans la durée, pas les traiter par épisodes

→ Aisance avec les outils numériques de suivi médical (DMP, logiciels métier, e-prescription)
