Bij ons draait ambulante begeleiding om groei, vertrouwen en zelfstandigheid. Als ambulant begeleider help je cliënten met GGZ-problematiek, verslavingsproblematiek, psychosociale uitdagingen en/of cognitieve achteruitgang om weer grip te krijgen op hun leven. Samen werk je aan doelen die hen sterker maken en stap voor stap dichter bij zelfstandigheid en maatschappelijke participatie brengen.

Wie ben jij?

Als ambulant begeleider sta jij in dienst van de cliënt. Jij weet dat echte verandering begint bij vertrouwen, en je gebruikt verschillende rollen en gesprekstechnieken om cliënten te motiveren, te ondersteunen en richting te geven. Je schakelt soepel tussen luisteren, bewust worden en prikkelen, altijd met het belang en de regie van de cliënt voorop.

Naast jouw begeleidende rol ben je ook administratief onderlegd: je werkt methodisch, stelt begeleidingsplannen op, rapporteert zorgvuldig en evalueert voortgang. Zo zorg je ervoor dat iedere stap inzichtelijk en effectief is.

Je maakt onderdeel uit van een betrokken team van persoonlijk begeleiders/ambulante begeleiders, een casemanager en een teamleider. Samen leren we van elkaar, ontwikkelen we onze dienstverlening en zorgen we ervoor dat cliënten de best mogelijke ondersteuning krijgen.

Zo zorgen wij voor jou

  • Een salaris volgens de cao VVT, FWG 45; tussen de € 3.592 en € 4.245 bruto per maand op basis van 36 uur per week;
  • Een contract voor 7 maanden, met de mogelijkheid voor verlenging of vast contract;
  • Goede secundaire arbeidsvoorwaarden zoals: 8% vakantiegeld, eindejaarsuitkering van 8,33% en een reiskostenruilregeling;
  • Laptop en telefoon;
  • Ruimte om je verder te ontwikkelen. We kijken samen naar leer- en ontwikkelmogelijkheden;
  • Mogelijkheid om mee te doen aan sportactiviteiten, zoals bootcamp en yoga. Maar ook workshops over bijvoorbeeld verminderen van stress of over de overgang.
  • Ondersteuning van een betrokken team waarin samen leren en groeien centraal staat.

Wat ga je doen?

  • Inventariseren van de ondersteuningsbehoeften van cliënten, en samen concrete doelen stellen;
  • Begeleiden van cliënten richting zelfstandigheid en maatschappelijke participatie;
  • Creëren van een veilige, vertrouwde omgeving waarin groei centraal staat;
  • Rapporteren, evalueren en methodisch werken aan de begeleidingsplannen;
  • Intensief samenwerken met interne collega’s en externe ketenpartners;
  • Bijdragen aan de ontwikkeling van ons team en samen leren en vernieuwen.

Dit neem je mee;

  • Een afgeronde MBO-4 opleiding (MMZ, SPW, Social Work of Persoonlijk Begeleider);
  • Ruime kennis en ervaring met GGZ- en verslavingsproblematiek, psychiatrische ziektebeelden, en psychosociale problematiek;
  • Beschikbaarheid voor 16 tot 24 uur per week; werkdagen/tijden kunnen in overleg:
  • Ervaring om verschillende rollen en gesprekstechnieken effectief in te zetten (zoals bewust worden en prikkelen);
  • Je hoofd kunnen koel houden in onverwachte situaties;
  • Administratieve en methodische vaardigheden;
  • Stressbestendigheid en flexibiliteit;
  • Rijbewijs B en eigen auto.
