Description du poste :

Services SFT recrute activement un(e) agent(e) de relations humaines (ARH) qualifié(e) et polyvalent(e) pour combler un mandat d'envergure à temps complet. Le besoin est partagé entre le Centre de santé de Chibougamau et le Centre de santé René-Ricard de Chapais.

Vous interviendrez au sein de deux départements clés en mission CLSC : Adaptation et soutien en déficience (DI-DP) ainsi que Jeunes en difficultés (JED - 6 à 17 ans). Il s'agit d'un horaire stable du lundi au vendredi, idéal pour un(e) professionnel(le) souhaitant vivre une expérience enrichissante dans le Nord-du-Québec tout en bénéficiant de conditions logistiques entièrement prises en charge.

Note importante : L'établissement privilégie activement les ressources en mesure de combler le besoin en entier.

Détails du contrat :

  • Période du besoin : Du 12 octobre 2026 au 12 avril 2027 (Mandat de 6 mois)
  • Orientation obligatoire : 1 jour d'orientation prévu au début du mandat
  • Quart de travail : Jour uniquement
  • Horaire type : Lundi au vendredi, 08 h 00 à 16 h 00 (comprenant 1 h de repas non facturable)
  • Garde : Aucune garde requise
  • Processus de sélection : Une rencontre Teams de validation sera planifiée avec le/la gestionnaire de l'établissement pour la personne sélectionnée.
  • Type d'emploi: Temps plein – 35 h / semaine.

Responsabilités:

  • Assurer l'évaluation psychosociale, l'orientation et l'intervention de première ligne (mission CLSC) auprès de la clientèle jeunesse âgée de 6 à 17 ans.
  • Concevoir, appliquer et évaluer les plans d'intervention pour les jeunes vivant avec une déficience intellectuelle (DI), un trouble du spectre de l'autisme (TSA) ou une déficience physique (DP).
  • Intervenir auprès des jeunes en difficultés (JED) et de leur famille afin de stabiliser les situations de crise ou de vulnérabilité.
  • Collaborer étroitement avec les équipes multidisciplinaires, les écoles et les partenaires communautaires des secteurs de Chibougamau et Chapais.
  • Documenter rigoureusement les dossiers cliniques en utilisant les logiciels I-CLSC et Crystal-Net.

Exigences de candidature:

Pour que votre dossier soit soumis, vous devez obligatoirement valider les critères suivants :

  • Scolarité : Détenir un baccalauréat universitaire dans une discipline des sciences sociales appropriée (travail social, psychoéducation, criminologie, psychologie).
  • Expérience requise : Cumuler un minimum de 2 ans d'expérience récente auprès de la clientèle jeunesse DI-TSA-DP et JED (6-17 ans) en mission CLSC.
  • Systèmes informatiques : Maîtrise obligatoire des logiciels I-CLSC et Crystal-Net.
  • Mobilité : Détenir un permis de conduire valide ET posséder un véhicule automobile (obligatoire pour couvrir les deux installations de Chibougamau et Chapais).
  • Documents de conformité : CV à jour, diplôme universitaire et Plumitif (antécédents judiciaires) datant de moins de 3 mois.
