Nous recherchons actuellement un(e) Agent(e) de relations humaines pour combler un mandat de longue durée combinant l'intervention psychosociale et le soutien à l'adaptation.

Description du poste

Intégrez une équipe pluridisciplinaire dynamique et engagez-vous dans un mandat à fort impact social au sein des communautés de la région. Vous partagerez votre temps et vos interventions entre deux installations clés : le Centre de santé de Chibougamau et le Centre de santé René-Ricard (Chapais).

Votre rôle couvrira deux départements d'intervention essentiels en mission CLSC :

  • Adaptation et soutien en déficience
  • Jeunes en difficultés

Détails du mandat :

  • Période du besoin : Du 19 octobre 2026 au 13 avril 2027 (Mandat de 6 mois).
  • Horaire type : Temps complet, du lundi au vendredi, de 8h00 à 16h00 (incluant 1h00 de pause repas).
  • Disponibilité requise : Quart de Jour uniquement.
  • Garde (On-Call) : Non.
  • Orientation : Une journée d'orientation (1 jour) est entièrement prévue et requise à votre arrivée.

Vos responsabilités

  • Assurer des fonctions de conception, d'évaluation, d'orientation et d'accompagnement psychosocial auprès d'une clientèle jeunesse (6 à 17 ans) présentant des problématiques de comportement ou d'adaptation (JED).
  • Concevoir, appliquer et évaluer des plans d'intervention adaptés pour la clientèle vivant avec une déficience intellectuelle (DI), un trouble du spectre de l'autisme (TSA) ou une déficience physique (DP).
  • Effectuer des suivis de proximité et des visites dans le milieu de vie des usagers au besoin.
  • Utiliser rigoureusement les systèmes d'information cliniques en place pour la tenue des dossiers.

Profil recherché

  • Diplôme requis : Baccalauréat universitaire de premier cycle pertinent (Travail social, Criminologie, Psychoéducation, Psychologie ou sciences sociales connexes).
  • Expérience exigée : Minimum de 2 ans d'expérience consolidée auprès de la clientèle jeunesse DI-TSA-DP et de la clientèle jeunesse JED (6-17 ans) en contexte de mission CLSC.
  • Systèmes informatiques : Maîtrise essentielle des logiciels cliniques I-CLSC et Crystal-Net.
  • Exigences logistiques : Posséder un permis de conduire valide ainsi qu'un véhicule automobile requis pour assurer vos déplacements professionnels entre les sites de Chibougamau et Chapais.
  • Savoir-être : Autonomie clinique, rigueur dans le suivi des dossiers, aisance avec les outils informatiques et fortes compétences interpersonnelles.
