The posting

Cura of Le Sueur is looking for a compassionate and motivated Social Services Coordinator to join our team!

This role is a great fit for someone who enjoys building relationships, advocating for others, and helping residents and families navigate the transitions that come with skilled nursing and long-term care.

As our Social Services Coordinator, you will be an important connection between residents, families, and the interdisciplinary team. You will get to know our residents personally, understand what matters to them, advocate for their needs, and help ensure they feel supported throughout their stay with us.

We are looking for someone who is approachable, organized, compassionate, and confident communicating with a variety of people. You should be comfortable having difficult conversations when necessary while still bringing empathy, professionalism, and a resident-first mindset to your work.

What You'll Do

  • Build strong, trusting relationships with residents and their families
  • Serve as an advocate for resident needs, preferences, choices, and rights
  • Help new residents and families navigate the transition into our facility
  • Complete psychosocial assessments and required social services documentation
  • Participate in care conferences and collaborate with the interdisciplinary team to develop individualized plans of care
  • Coordinate discharge planning and connect residents with community resources, services, equipment, and transportation
  • Maintain regular communication with residents and families and assist with questions or concerns
  • Support residents experiencing emotional, behavioral, social, or adjustment-related challenges
  • Coordinate behavioral health, mental health, and other supportive services when needed
  • Assist with advance directives and other resident planning needs
  • Help resolve resident and family concerns and ensure appropriate follow-up
  • Collaborate closely with nursing, therapy, MDS, activities, dietary, providers, and facility leadership
  • Support compliance with resident rights and applicable state and federal requirements
  • Participate in QAPI, quality improvement, and survey readiness activities
  • Help create an environment where residents feel heard, respected, safe, and supported

Requirements

  • Background in social work, human services, psychology, sociology, healthcare, or a related field is preferred
  • Experience in skilled nursing, long-term care, healthcare, case management, or social services is preferred
  • Strong communication and relationship-building skills
  • Ability to remain professional and solution-focused during challenging situations
  • Strong organization and follow-through
  • Ability to manage multiple priorities throughout the day
  • Comfortable working both independently and as part of an interdisciplinary team
  • Understanding of resident rights, community resources, and long-term care regulations is a plus
