ZAS, De drijvende kracht voor gezondheidszorg in beweging. Drijf je mee?

Achter elke jongere schuilt een uniek verhaal. Als maatschappelijk werker binnen UKJA, op de FOR-K en SGA-afdeling, begeleid je jongens van 12 tot 18 jaar met externaliserende gedragsproblemen of seksueel grensoverschrijdend gedrag en hun netwerk. Met jouw sociale expertise bouw je bruggen, maak je rechten toegankelijk en zoek je mee naar oplossingen tijdens hun residentiële behandeling.

OVER DE JOB

  • Sociaal onderzoek & advies – Je voert sociaal onderzoek uit en stelt passende dienstverlening voor, afgestemd op de patiënt en zijn context.
  • Begeleiding naar ontslag – Je begeleidt patiënten naar een haalbare ontslagsituatie en zorgt mee voor de nodige vervolghulp.
  • Psychosociale ondersteuning – Je bevordert het psychosociale welzijn van patiënten en werkt multidisciplinair samen.
  • Sociale verslaggeving – Je stelt duidelijke sociale verslagen en adviezen op, ook voor externe instanties.
  • Netwerk & samenwerking – Je onderhoudt contacten met externe hulpverleners en organisaties voor doorverwijzing en samenwerking.
  • Dossieropvolging – Je coördineert individuele dossiers en begeleidt patiënten en hun context in sociaal-maatschappelijke zorg.

OVER JOU

  • Diploma – Je hebt een bachelor Sociaal Werk of Sociaal Verpleegkundige en kent het hulpverleningsveld.
  • Sociale expertise – Je kent sociale voorzieningen, rechtenonderzoek en sociale wetgeving en hebt interesse in ethische vraagstukken.
  • Psychische zorg – Je hebt interesse in psychische ziektebeelden en wilt je hierin verder verdiepen.
  • Digitaal vaardig – Je hebt een goede basiskennis van Microsoft Office en andere informaticatoepassingen.
  • Communicatief – Je communiceert uitstekend in het Nederlands en beschikt over goede interculturele en administratieve vaardigheden.
  • Zelfstandig & gestructureerd – Je werkt autonoom, stelt prioriteiten en pakt problemen oplossingsgericht aan.
